If It Is The Disabled Veteran Completing The Application They Must Submit The Following:
Documentation from the US Department of Veterans’ Affairs for the current assessment year showing combined disability percentage AND one of the following: FORM  DD214 OR Separation of Service from the War Department (prior to 1950) OR Certification of Military Service Form (this form may show the combined disability percentage) OR Illinois Drivers License or ID card showing a Veteran’s Designation

If it is a Surviving Spouse of a Disabled Veteran the two documents above are required
AND A copy of the veteran’s marriage certificate  AND A copy of the veteran’s death certificate 

If it is a Surviving Spouse of a Veteran killed in the line of duty, ONLY the following is required:
DD Form 1300 Report of Casualty (if killed in the line of duty)
